Spravato® (esketamine) therapy in Grand Rapids
SmartMinds Psychiatry is a certified REMS center offering Spravato® (esketamine) therapy for treatment-resistant depression, depression that has not responded to other treatments.
About Spravato
Spravato is an FDA-approved prescription nasal spray for adults with treatment-resistant depression. Because of its risks, it is available only through a restricted safety program called a REMS, and each dose is taken in a certified treatment center under a healthcare provider's supervision.
Patients are observed after each dose and should not drive afterward. Your care team will explain exactly what to expect and how long to plan for each visit.
What is treatment-resistant depression?
Treatment-resistant depression generally describes major depression that has not improved enough after trying at least two different antidepressants at an adequate dose and for long enough. It is common, and it does not mean nothing can help. It means it may be time to look at other options with your provider.
What to expect at a Spravato visit
- You give yourself the nasal spray in our office, under the supervision of our clinical team.
- You stay with us to be monitored afterward, usually for at least two hours, until your provider says it is safe to leave.
- Arrange a ride home: you should not drive or operate machinery until the next day after a restful sleep.
- Treatment typically starts with more frequent visits that are spaced out over time. Your provider will explain your schedule.
Is Spravato right for me?
Spravato is not right for everyone. Your provider will review your history, your current medications and your goals before recommending it. A psychiatric evaluation in Grand Rapids is a good place to start.
Spravato is part of a wider treatment plan. Many patients continue psychiatric medication management and benefit from therapy and counseling alongside it.

Spravato contains esketamine, which is related to ketamine. It is a specific FDA-approved nasal spray given only at certified treatment centers under the Spravato REMS program.
Why do I have to stay in the office after each dose?
Spravato can cause sleepiness, dizziness, a feeling of being disconnected, and changes in blood pressure. Monitoring after each dose is required by the REMS program to keep you safe.
Can I drive myself home?
No. Plan for someone to drive you home. You should not drive or operate machinery until the next day after a restful sleep.
